Terms of Service
These terms govern use of FormsMD (the "Service") provided at formsmd.ca and app.formsmd.ca. By creating an account you agree to them on behalf of yourself and your practice.
1. Who may use the Service
FormsMD is offered to physicians licensed to practise in Canada and to staff acting under their direction. Each account is approved before it becomes active, and we may ask for proof of licensure.
2. Clinical responsibility
The Service drafts forms and letters from the information you provide. Every draft is a starting point. The physician remains solely responsible for reviewing, correcting, signing and sending each form, and for the clinical accuracy of its contents. FormsMD does not provide medical advice.
3. Plans, trial and billing
- New accounts receive a 30-day free trial. No card is required to start.
- Paid plans are billed monthly in advance, in Canadian dollars plus applicable taxes, from the date the plan begins. Each plan includes a monthly allowance of AI-drafted forms; the allowance resets on your billing date and unused forms do not carry over.
- You may change or cancel your plan at any time from Settings. Cancellation takes effect at the end of the current billing period; we do not issue refunds for partial periods.
- If a payment fails, we will notify you and retry. Access may be paused until the balance is paid.
4. Acceptable use
You agree to use the Service only for legitimate medical documentation, to keep your sign-in credentials private, and not to attempt to access other practices' data, reverse-engineer the Service, or use it to send unsolicited communications.
5. Your data
You own the content you enter. We process it only to provide the Service, as described in our Privacy Policy. You can export your forms at any time.
6. Availability
We aim for continuous availability but do not guarantee it. Planned maintenance is announced in advance where practical. Because faxes and forms may be time-sensitive, keep an alternative means of sending urgent documents.
7. Limitation of liability
To the fullest extent permitted by law, FormsMD's total liability for any claim arising from the Service is limited to the fees you paid in the three months before the claim arose. We are not liable for indirect or consequential losses.
8. Changes
We may update these terms; material changes will be announced in the application at least 30 days before they take effect. Continued use after that date constitutes acceptance.
9. Governing law
These terms are governed by the laws of Ontario and the federal laws of Canada applicable therein.
