Safe Scribe: the AI medical scribe that never leaves Canada.

Record the visit or dictate afterwards. The transcript and the clinical note are written by the best medical AI models available — on our own servers in Kitchener, Ontario, operated by a Canadian company — then double-checked by a second model. Nothing goes to a public AI service. Nothing crosses the border. Built into FormsMD; any browser, nothing to install.

Patient visit · 6 min 12 s · two speakers · note ready 48 s after Stop

S: 52-year-old man, follow-up of hypertension. Reports occasional morning headaches for two weeks; denies chest pain or visual change. Taking ramipril 10 mg daily; missed two doses last week.

O: BP 148/92 seated, repeat 144/90. Weight not discussed.

A: Hypertension, suboptimally controlled; adherence likely contributing.

P: Continue ramipril 10 mg daily. Home BP log twice daily for two weeks. Electrolytes and creatinine. Review in three weeks.

Items to confirm: none.

Fictional example. Checked twice — the physician reads it over, changes anything, and files it.

How the visit becomes the note

Four steps, all on our hardware in Ontario. You see the progress on screen and get the note back in about a minute after you press Stop.

1

Record, or upload

Press Record in the browser during the visit, dictate afterwards, or upload a recording from your phone. The browser asks once to use the microphone.

2

Transcribed by medical speech models

Dictations go to a speech model trained on physician dictations; visits to the best conversational model, with the two speakers separated. Your patient's medications are expected words.

3

Written, then checked twice

The note is drafted in your template by the top-scoring medical model in its class, then a second model reads the recording against the draft and corrects drug names, doses, numbers, left and right, and anything the recording doesn't support.

4

Review, edit, file

Edit on screen, save to the chart, download as PDF or Word on your letterhead — or hand it to SafeAI for the referral letter or the insurer's answer.

The best medical models, run here — not in someone else's cloud

We pick the models by what independent medical benchmarks say, keep them current as better ones appear, and run every one of them on our own GPUs in Kitchener, Ontario. You never choose or update a model; you just get the best note we can write.

Speech built for medicine

Dictations are transcribed with Google's medical speech model, trained on thousands of hours of physician dictations — a fraction of the word errors of general-purpose transcription on drug names and doses. Patient visits use the best open conversational speech models, with speakers separated so the patient's words stay history and the clinician's stay findings.

Two independent minds on every note

The note is drafted by the model that scores highest in its size class on independent medical benchmarks, then checked by a second model from a different family. A model can't see its own blind spots; a different one can. Both run locally.

It knows your words

The patient's medication list and allergies, your specialty's terms and the names you dictate often are given to the listener as expected words, so metoprolol is metoprolol and Dr. Nguyen is Dr. Nguyen.

Your template, your note

SOAP, DAP, a progress note or your own template, with free-hand directions saved once. Doses and numbers are copied as said, "unclear in recording" replaces a guess, and every note ends with an "Items to confirm" line.

Processed and saved in Canada, by a Canadian company

Most AI scribes send the recording to a US cloud. Safe Scribe doesn't send it anywhere: the only computers that ever hear it are ours, in Ontario.

Recorded in your browser

Encrypted in transit to our server in Kitchener, Ontario. No third-party transcription service, no public AI, no border crossing.

Kept as long as you say

The recording is deleted automatically after the retention you choose — 30 days by default, or the moment the note is written. Transcripts and notes stay encrypted under your practice's own key, and are yours to delete at any time.

Built for Canadian privacy law

What you get

Patient visit or dictation

Record the conversation and the speakers are told apart, or dictate the note yourself after the visit — same button.

Any device, no install

Chrome, Edge, Firefox or Safari on a phone, tablet or computer. Upload a recording from a phone recorder too — MP3, M4A, WAV, WebM, up to two hours.

The transcript, verbatim

Exactly what was heard, speaker by speaker, on screen beside the note and downloadable as text, Word or PDF — the evidence when a note needs checking.

Edit and export

Change any wording on screen, see the note as it prints on your letterhead, download PDF or Word, or save it straight to the chart.

Into the paperwork

Hand the note to SafeAI for the referral letter, the consult reply or the insurer's answer — from the same screen.

Timed, every time

Each note shows how long the recording was and how long Safe Scribe took to transcribe, write and check it — usually about a minute.

Questions physicians ask about Safe Scribe

Is Safe Scribe an app I have to install?

No. It runs in the browser you already use — Chrome, Edge, Firefox or Safari, on a phone, tablet or computer. Open app.formsmd.ca, press Record, and the browser asks once to use the microphone.

Where is the recording processed and stored?

On FormsMD's own servers in Kitchener, Ontario, operated by a Canadian company. The audio is transcribed and the note is written on hardware we run — never sent to OpenAI, Google's cloud, Anthropic or any other outside service — and nothing crosses the border. The recording is deleted automatically after the retention period you set (30 days by default, or as soon as the note is written); the transcript and note stay under your practice's encryption key.

Which AI models does Safe Scribe use?

The best available for the job, and we change them as better ones appear — you never have to do anything. Dictations are transcribed with Google's medical speech model, trained on physician dictations; patient visits with the best open conversational speech models, with the two speakers separated. The note is drafted by the top-scoring open model in its size class on independent medical benchmarks, then checked by a second model from a different family. Every model runs on our own GPUs in Ontario.

What does "checked twice" mean?

After the first model drafts the note, a second model reads the transcript and the draft and returns a corrected note: drug names, doses, routes and frequencies as said, numbers copied exactly, left and right, dropped negations, and anything the recording doesn't support removed. Two independent models have to agree before the note reaches you.

Does it know my patients' medications?

Yes. When a patient is selected, their medication list and allergies — and the terms you add in Settings — are given to the speech model as expected words, so metoprolol comes out as metoprolol and dupilumab as dupilumab.

Can I edit the note and the format?

Always. Pick SOAP, DAP, a progress note or your own template; add free-hand directions once in Settings. The note is yours to edit on screen, download as PDF or Word on your letterhead, or save to the chart — and SafeAI can turn it into the referral letter or the insurer's answer from the same screen.

Is Safe Scribe ready for patient care?

Safe Scribe is in beta. It can mishear a drug name or a number, which is why every note ends with an "Items to confirm" line and why you review every note before it goes in the chart. It never files, sends or signs anything on its own.

What does it cost?

Safe Scribe is $15 a month (CAD, plus HST) on any FormsMD plan — Starter, Pro or Clinic — switched on from your Subscription page. On Pro and Clinic every note is checked twice by a second model. Every plan starts with a 30-day free trial; no card to start.Nothing during the FormsMD beta: Safe Scribe is included free on every account, switched on from your Subscription page.
